This plan offers first dollar coverage of up to $500, followed by an individual deductible of $1,000 and a family deductible of $1,500. 

HealthChoice Basic Alternative

This plan offers first dollar coverage of up to $250, followed by an individual deductible of $1,250 and a family deductible of $1,750.
